About the Position:

  • Manage the administrative duties and functions of the medical office, including completion of all required paperwork within designated time frame.
  • Consult and communicate with pharmacies regarding client medications and documentation of medications.
  • Ensure that immediate client needs (e.g., prescribed medications, allergy management, dietary needs) are adequately planned for, well communicated, and in place prior to new client admissions.
  • Ensure the establishment of primary medical, dental and obstetrical providers for all new clients entering the program, including setting initial appointments and sharing (client-released) relevant medical history with new providers.
  • Maintain and regularly update emergency medical information for all enrolled clients, ensuring easy access for all staff.
  • Ensure that all follow-up, sick, and well-care visits for clients and their children are completed in a timely fashion.
  • With nursing supervision and as dictated through written provider orders, dispense prescribed and over the counter medications.
  • Maintain strict accuracy of the Medication Administrative Record (MAR), including documentation of daily medication dispensing, as well as timely recording of all medication changes and additions.
  • Ensure timely communication of medication additions and changes to appropriate staff.
  • Monitor all medications on a regular basis to ensure timely prescription refills in advance of actual need. Ensure effective, collaborative communication with prescribers and pharmacy personnel throughout this process.
  • Work directly with clients to assist in building skills to schedule their medical appointments, renew prescriptions, and advocate for personal and family medical needs.
  • Complete all required paperwork within established timelines, including, but not limited to discharge summaries and daily client and parenting logs as appropriate.
  • Provide transportation and accompany clients to medical and dental appointments, assist clients with information integration, and incorporate new medical information into existing client treatment planning. 
  • When appropriate, attend necessary meetings pertaining to current client needs, as well as admission and discharge planning meetings.
  • Ensure responsible fiscal management through such practices as careful management of existing stock.
  • Demonstrate the ability to manage time effectively, work independently, and as a team member.
  • On-call duties as assigned.
  • Other duties as assigned.

 What We Look For:

  • At least two years of experience working with people who are facing significant emotional, social and familial challenges.  
  • Relevant education or experience in medical case management role.
  • At least one year of experience with an adolescent/young adult population, preferably in a residential setting.
  • Excellent interpersonal, verbal and written communication skills.
  • Possess a cohesive value system that includes a nonjudgmental attitude.
  • Willingness to work irregular hours at times to accommodate the needs of a 24-hour facility.  Weekend hours required.
